This self-test is not intended
to be used for the diagnosis or treatment of any medical or
emotional condition. We advise you to consult your doctor about
any concerns you may have regarding this questionnaire. The
purpose of this survey is to give you some insight into whether
or not you may have a problem with Alcohol and/or Drugs.
Do you lose time from work due to your drinking
or drug use?
Is your drinking/drug use making your home life unhappy?
Do you drink or drug because you are shy with other people?
Is drinking or drug use affecting your reputation?
Have you ever felt remorse after drinking or drug use?
Have you gotten into financial difficulties because of your
drinking or drug use?
Do you turn to lower companions and inferior environment when
drinking or using drugs?
Does your drinking or drug use make you careless of your family's
welfare?
Has your ambition decreased since drinking or using drugs?
Do you crave a drink or a drug at a definite time daily?
Do you want a drink or drug the next morning?
Does drinking or drug use cause you difficulty in sleeping?
Has your efficiency decreased since drinking or using drugs?
Is drinking or drug use jeopardizing your job or business?
Do you drink or use drugs to escape from worries or troubles?
Do you drink or use drugs alone?
Have you ever had a complete loss of memory as a result of your
drinking or drug use?
Has your physician ever treated you for drinking or drug use?
Do you drink or use drugs to build up your self confidence?
Have you ever been in a hospital or institution as a result
of drinking or drug use?
If you have answered YES to any one of the
questions, there is a definite warning that you may be an alcoholic.
If you answered YES to any two, the chances
are that you are an alcoholic.
If you answered YES to three or more, you
are definitely an alcoholic.

whether or not a patient is alcoholic.)

(I rated the John Hopkins questions as
a 5-star test. The CAGE test (below) is old and for obvious
reasons in my book,didn't help me consider myself an alcoholic.

Here is the CAGE test, you can go
ahead and take it now:
1. Have you ever felt you should cut down
on your drinking?
2. Have people annoyed you by criticizing your drinking?
3. Have you ever felt bad or guilty about your drinking?
4. Have you ever had a drink first thing in the morning to steady
nerves or get rid of a hangover?
Scoring: Two or three affirmative answers
is highly suspicious, and would indicate that you are an poised
to have a drinking problem if you don’t already. If you
answer “yes” to any of these, and you don’t
think you have a problem with drinking, I think you are in denial
or unaware of how close you are to having a life-altering problem.
If you give a “yes” to all four questions, you’re
pinpointed as an alcoholic.
By the way, the acronym CAGE comes from
four words in the questions in this order: “Cut down”,
“Annoyed”, “Guilty”, and “Eye
Opener”. “Eye opener” isn’t actually
in the words of number four, but I think that it is the biggest
indicator of problem drinking and alcoholism.
